CYGHA Injured Players Pathway

What forms to use and when?

       All serious injuries require a OWHA Injury Report

       Concussions/Head Injuries follow Hockey Canada Head Injury Guidelines for management and return-to-sport which include Medical Assessment Letter and Medical Clearance Letter

       Non-head injuries requiring provider assessment require a clearance to participate form

       Hockey Canada has concussion resources for trainers, players and parents

Process

1.      A player is injured at practice or in a game

2.      Trainer provides assessment and activates EAP if necessary

3.      If the injury is serious, OWHA Injury Report is completed by trainer and provided to parents
       (a copy can be uploaded by trainer to player profile in ePACT)

If no concussion is suspected:

4.     Player should be evaluated by a medical provider, the provider section of the OWHA Injury Report should be completed
      (with diagnosis and anticipated return) and uploaded to ePACT

5.     When player is able to return; clearance to participate form should be completed by the provider and uploaded to ePACT.

If concussion is suspected:

4. If head injury is suspected, Hockey Canada Head Injury Medical Assessment Letter is provided to parents to be completed by a medical provider who will either clear the player (no concussion), identify other injury (no concussion) or diagnose a concussion and initiate concussion protocol attached in the Medical Assessment Letter  This form should then be uploaded to ePACT by parents

5. Once player is at Step 4 of concussion protocol they can be cleared for participation in practices by their provider. This requires their provider to sign the Medical Clearance Letter. Once signed, this must also be uploaded to ePACT.

Other important information:

*** If a player is suspected of having a concussion, it may be beneficial to also bring forms needed by their school board to also be signed by the provider to facilitate any necessary in school accommodations ***

*** If a player has suffered a concussion (outside of activities associated with CYGHA), they must inform their trainer and be cleared to return to sport, the Medical Clearance Letter can be used and should be uploaded to ePACT ***

*** If a player has suffered an injury that keeps them from practice and games (outside of activities associated with CYGHA), they must inform their trainer and be cleared to return to sport, the clearance to participate form should be completed by their providers and uploaded to ePACT. ***

ePACT

The purpose of using ePACT is to provide a digital and secure platform for players personal health information. Parents should be able to upload all completed forms to the file sections of their player’s profile. In the event this is not possible, due to technical difficulties or the ePACT site being down, trainers can review the forms in person and attempt to upload them using their admin ePACT (file section of the individual player). Alternatively, the trainer can review the completed form in person and send an email to the coach and other trainers copying the parents attesting the forms are complete and the player is safe to return to play (without including medical information).

The CYGHA strongly discourages Trainers, Coaches and other CYGHA representatives from sharing or possessing the personal health information of players.

All Trainers must have:

Trainer Certification:  CYGHA reimburses 100% of certification - send receipt to the office
Trainer certification is an online course.  It is valid for 3 years at which time the trainer must Re-Certify.

Vulnerable Sector Check (VSC) - CYGHA reimburses 100% - send receipt to the office

Please use the link below to apply for your VSC.  When picking up your VSC, you must provide a letter from the CYGHA verifying your volunteer position.  Please contact the office ([email protected]) to provide you with a letter

To upload your VSC you must use this link Ontario Screening Submission Portal 

Your VSC is valid for 3 years. EVERY year you must still use the above link and answer the questions.  This acts as a criminal declaration.
